Smart Shoppers Savings Clu

This company sent me statement stating that I owe 201.45 for a "voucher" that I never ordered online. I noticed that on the statement that there is not contact person or signature to communicate my dipute. When I called to talk to someone, I was given another number, I called it and the person on the other line had my information of the screen. How did he know I had called a previous person? Another interesting thing, is that one the statement there are two amounts that I can "settle" with and the only way I can settle is by a credit card payment. I told him that I would send the payment by check, but he refused because he said that the mail would be sent to Virgina, and on my statement, the address states Arizona. What is going on?.... If anyone can help, please let me know or direct me to a contact person.
